Explosion at varnish factory occurred when toluene was added to solvents in a drum; one worker died and another was injured.
The toluene manufacturer failed to provide adequate warnings and instructions for using toluene. The warnings and instruction failed to alert users of the hazards of the toluene with regard to grounding requirements, ventilation requirements, the dangers of mixing toluene with other chemicals and the dangers of empty containers. The only apparent warnings or instructions issued in connection with toluene were contained in a material safety data sheet, that may not have ever been provided to the workers’ employer.
