A manufacturer of highly flammable aerosol sprays altered a compressor which vented gas into an occupied production area.

 

The gas was ignited when the compressor was shut off. Several people died and many others were seriously injured when the explosion gutted the work area. Investigation revealed that inadequate equipment and training, among other things, contributed to the explosion.
