Toys R Us recalled the Imaginarium 5-Sided Activity Center due to choking hazards. It was reported that the small wooden knobs attaching the xylophone keys can detach, presenting a choking hazard to young children. Toys R Us has received at least eight reports of the knobs separating.
The recall by Toys R Us includes the multi-sided center with two triangle-shaped ends, one with a wooden multi-colored xylophone, the other with a mirror. The three sides are square-shaped and include moveable block letters, rotating gears, and sliding shapes. This toy was sold across the United States for $25.
